For commercial and Individual Exchange plans
To help meet the requirements of Colorado state regulation 4-2-80, all health care professionals who are in-network for UnitedHealthcare commercial plans and UnitedHealthcare Individual Exchange plans¹ in Colorado are required to provide us with information about their anti-bias and cultural competency training.

- If applicable, the expiration date of the certification
