Maryland: Support your patients through MOM
Get help for your pregnant and postpartum patients diagnosed with opioid use disorder
Effective Jan. 1, 2023, pregnant and postpartum patients enrolled in Medicaid have an opportunity to participate in the Maternal Opioid Misuse (MOM) model through the Maryland Department of Health.
Funded by the Centers for Medicare & Medicaid Services (CMS) Innovation Center, the MOM model works collaboratively with Medicaid managed care organizations (MCOs) to provide enhanced case management services that focus on improving care for pregnant and postpartum patients diagnosed with opioid use disorder (OUD).
What this means for you
You can help support your patients who are potential and/or enrolled MOM participants through the following:
- Potential participants
- Health care professionals can refer pregnant and postpartum Medicaid participants diagnosed with opioid use disorder (OUD) to program resources by helping them complete and submit the Maryland MOM Case Management Services request and consent form
- Somatic therapists can also complete the Maryland Perinatal Risk Assessment form
- Enrolled participants
- Health care professionals can communicate activities with MOM case managers, including missed appointments, loss to follow-up, positive drug and/or sexually transmitted disease screens, making referrals to specialists, etc.
- Health care professionals who treat substance use disorder can provide enrolled participants with a consent form for data sharing under the 42 CFR Part 2 Release of Information process
How MOM works
Dedicated MOM case managers will create individual care plans for MOM participants, including a minimum of 1 health care visit every month. Primary care providers can review their patients’ care plans and find the name and contact information of the MOM case manager through CRISP, the designated health information exchange system for Maryland.
What the MOM model funds
Funding provided to MCOs helps improve case management programs by focusing on:
- Increasing utilization of physical and behavioral health care, including medication for OUD
- Ensuring access to community resources for those identified as being at risk with social determinants of health
