CPT III code policy update starting April 1
UnitedHealthcare West and Oxford plans will require prior authorization for most CPT III codes
Starting April 1, 2023, we’ll require prior authorization for most CPT® III codes, also known as T codes, for UnitedHealthcare Oxford and UnitedHealthcare West plan members.

Our medical policy will be updated on April 1, 2023, with an updated list of category CPT III codes at UHCprovider.com/policies.
Submit a prior authorization
- Sign in on the UnitedHealthcare Provider Portal Resources page.
- Click on the Prior Authorization tool
- Select Create a new notification or prior authorization request
- Enter the required information on the submission site
