Maryland Medicaid: Connecting at-risk moms with social services

Help reduce the risk of infant and maternal mortality

Did you know? Pregnant individuals who are enrolled in Medicaid must complete the Maryland Prenatal Risk Assessment (MPRA) form during their first prenatal visit at a managed care organization. This requirement by the Maryland Department of Health will help reduce the risk of infant and maternal mortality and diminish health care disparities.

How the program works

The Maryland Department of Health uses the MPRA to connect at-risk pregnant individuals with health and social services, such as the following:

  • Home visit services
  • Case management and care planning
  • Substance use disorder programs
  • Nutrition programs

What this means for you

With your help, at-risk patients can get the services they need beyond the care you provide. For each patient who completes an MPRA form, you can receive $40 reimbursement by taking these steps:

  • Complete the form with your patient during their first prenatal visit
  • Send the form within 10 days of completion to the local health department
  • Submit a claim with CPT® code H1000 for prenatal care, at-risk assessment

Claims must be submitted promptly and accurately to ensure payment.

Review the Maryland Department of Health MPRA flyer for more information.
