UnitedHealthcare D-SNPs are closing in Ohio

What you need to know about provider attestation for alternative plans

Starting Jan. 1, 2027, UnitedHealthcare will no longer offer Dual Special Needs Plans (D-SNPs) in Ohio. Patients currently enrolled in a D-SNP must select a new plan during this year’s annual enrollment period. Some of these patients may be eligible for a UnitedHealthcare Chronic Special Needs Plan (C-SNP). Provider attestation is required for enrollment.

What this means for you

CMS requires care providers to attest that a patient has one of the following qualifying chronic conditions to confirm eligibility for a C-SNP plan:

  • Diabetes mellitus
  • Chronic heart failure
  • Cardiovascular disorders (cardiac arrhythmias, coronary artery disease, peripheral vascular disease or valvular heart disease)

When patients select a UnitedHealthcare C-SNP, UnitedHealthcare will email the attestation form and submission information to your office. If we don’t receive a response to the email, we’ll follow up by fax and a phone call.

What is a C-SNP?

C-SNPs are Medicare Advantage plans that focus on people living with severe or disabling chronic conditions. These plans provide the same coverage as traditional Medicare (Parts A and B) but include prescription drug coverage (Part D) and additional benefits.

C-SNP benefits

  • Coordinated care among primary care providers (PCPs), specialists and facilities
  • Prescription drug coverage, often including medications specific to the chronic condition
  • Specialized services (e.g., home-delivered meals, wellness programs and disease-specific education and support)
  • Preventive care and routine exams (including vision and hearing benefits, depending on the plan)
  • Monthly credit for over-the-counter products, heathy food and home utilities for qualifying individuals

Who’s eligible for a C-SNP

To enroll, patients must:

  • Have a qualifying chronic condition as defined by CMS, such as diabetes, chronic heart failure, stroke, lung disease or end-stage renal disease
  • Meet any plan-specific requirements, which may include living in a certain area or being under the care of a PCP who can provide referrals to specialists
