Join the UnitedHealthcare network of health care facilities and hospitals. This page will walk you through the process of credentialing and provide resources.

Thank you for your interest in joining the UnitedHealthcare network. We’re here to help make the process as easy possible. Our improved onboarding tool, Onboard Pro, located in the UnitedHealthcare Provider Portal, is the first step to request participation in our network.

Create a One Healthcare ID

If you don’t already have a One Healthcare ID, you’ll need one to sign in to the UnitedHealthcare Provider Portal for Onboard Pro access.

You can view real-time status updates on your application to join the UnitedHealthcare network of hospitals and facilities in Onboard Pro. Once signed in, you'll be able to see the dashboard, which shows projected completion dates for both contracting and credentialing. If you need additional help, please connect with us 24/7 through chat.

No. All individual health care professionals associated with any participating UnitedHealthcare network of hospitals and facilities must go through the credentialing and contracting process. A facility contract does not include the health care professionals working there.

Yes. Your contract with us can be renegotiated or terminated. To start this process, please connect with us through chat in the UnitedHealthcare Provider Portal

Yes. Since Onboard Pro currently only handles single-state requests, you’ll need to submit individual applications for each state. 

If you received a letter stating that the market is closed — meaning UnitedHealthcare is not currently accepting applications for your specialty or provider type in your geographic service area — you may request a reassessment if you meet 1 or more of the following criteria:

  • You provide specialized care other providers in the area don’t provide in your geographic area
  • You’re acquiring a practice or health care business that is already in the UnitedHealthcare network, such as through a provider’s retirement, death, or the sale or transfer of ownership
  • Other reasons that may warrant additional reassessment of your request

If any of the above apply, you may request a reassessment by completing the reassessment application request form

How to submit
  • Once the form is completed, save the form to your files
  • Then, sign in to the UnitedHealthcare Provider Portal with your One Healthcare ID
  • Select the chat icon at the bottom-right corner of the page to connect with an advocate, 7 a.m.–7 p.m. CT

Please have the care provider’s full name, tax ID number (TIN) and National Provider Identifier (NPI) number ready when you start your chat session. If you have an assigned network contract manager, you can also contact them directly.

Recredentialing

UnitedHealthcare automatically starts the process when you approach the 3-year credentialing cycle. If you maintain a complete and current data attestation every 120 days, there's nothing more you need to do.

Additional state and federal credentialing requirements

UnitedHealthcare Credentialing Plan 2025–2027
