Changes to your demographic data submission process

For groups and practitioners, on Oct. 1, 2024, we’re removing email as an intake option to maintain your demographic data. We’ll also be removing access to the demographic and practice change request form.

Roster managed demographic updates are not affected by this change.

Moving forward, we ask that you submit your demographic updates through 1 of the following methods to ensure timely and accurate updates of your data:

  • Council for Affordable Quality Healthcare (CAQH) Provider Data Portal:  Allows you to access directory services and update profiles in real time. The data portal is designed to serve multiple payers, meaning any updates or attestations you make will be accessible to participating payers that you’ve authorized.
  • My Practice Profile:  Allows you to verify and attest to data accuracy for each health care professional associated with a specific TIN. You can find My Practice Profile within the UnitedHealthcare Provider Portal.
  • Roster Submission: If your demographic updates are currently submitted on a roster, please follow the roster process for your updates.

You only need to submit your demographic updates through 1 of the noted methods; there is no need to submit to multiple places. These methods will help us gather all the necessary information to promptly update your records and eliminate the need for further clarifications.

Why it matters

Please note that failure to submit your demographic information by 1 of the methods above may result in inaccuracies in our directory, a negative patient experience and delays in claims payment.  

Medicaid contracted health care professionals

If your state requires you to submit updates through a specific tool or process, continue to follow that process to keep your data updated with the state. You will also need to submit your updates through 1 of the options noted above.
