Ohio Medicaid: Time to revalidate your Medicaid agreement

Every 5 years, credentialed providers must submit an application to ODM

As of June 2023, all Ohio Medicaid credentialed providers must complete and submit a Medicaid Agreement Revalidation application every 5 years in the Ohio Medicaid Department (ODM) Provider Network Management (PNM) module. The month and year of renewal is based on the end date in their Ohio Medicaid Provider Agreement.

What this means for you

If your Medicaid agreement is up for renewal, you’ll receive a letter or email from the ODM 120 days before your agreement end date. The notice will include instructions and a link to begin the revalidation process. If you think you are due for revalidation but have not received a notice, please log in to the PNM module and verify that the primary contact information is accurate in accordance with your Ohio Medicaid Provider Agreement.

Ensure you’re in compliance

Failure to comply before your agreement end date may result in the termination of your provider status with Ohio Medicaid and UnitedHealthcare Community Plan of Ohio. Claims for services rendered after the termination date may be denied.
