Prepare for new Choice Plus network members

Beginning July 1, 2026, The Salvation Army – Central Territory members will have access to the Choice Plus network.

 

What this means for you

To view benefits, eligibility, prior authorization requirements and claim updates for these new members, please visit the UnitedHealthcare Shared Services Provider Portal, administered by UMR.

 

You don’t need to take any action during this transition. You can identify The Salvation Army – Central Territory members by looking at the following member ID card:

The Salvation Army – Central Territory members have access to Choice Plus (medical) and UnitedHealthcare/Optum Behavioral Health network benefits. To verify coverage, call 888-830-0179.

Prior authorization requirements

  • Call Chesterfield Resources at 800-305-4271
  • Required for all hospital admissions
  • Emergency admissions must be reported within 24 hours
