UHC One Care Medicare Advantage plans are Dual Special Needs Plans (D-SNPs) that contract with both Medicare and MassHealth (Medicaid) to provide benefits of both programs to enrolled members. The plans are available in 9 Massachusetts counties and are intended for eligible individuals ages 21 through 64 who have both Medicare and MassHealth (Standard).

Health Maintenance Organization (HMO) plans use a network of contracted local physicians and hospitals to provide member care. Generally, members must use these care providers to receive benefits for covered services, except in emergencies. The UHC One Care plans require referrals for specialty care.
