- Share your screening assessments using LOINC or ICD-10 Z codes
- When you identify an SDOH gap for your patient, submit your claims using CPT® or HCPCS intervention codes

Submit SDOH codes using either of the following digital solutions:
- Electronic data interchange (EDI) clearinghouse
- UnitedHealthcare Provider Portal
Connect your patient to resources
When you identify members with a social need and don’t have resource information readily at hand, we can help. Direct them to UnitedHealthcare Customer Service. Our team will connect members to community resources, if available.
